Couples Counseling

Insurances

Daniel Brake, LMFT-S is on panel for:

Insurance is accepted for individual counseling only. We do not accept any form of insurance for couples therapy, discernment counseling, or ACT Affair Recovery.

Keeley Arnecke, LMFT Associate is out of network for all insurance.

In-Network Benefits: For most plans, the session cost is between $10 – $30, and will be the same as seeing a general physician. If there is a deductible (there often is a separate deductible for mental health), the session cost is the “negotiated rate” set by the insurance company until that deductible is satisfied. Please be aware that to use your insurance benefit, you will have to receive a diagnosis code from the DSM-V. If you have any concerns regarding this, make your therapist aware of them prior to your first session.

Out-of-Network Benefits: Depending on your individual provider and policy, you may be able to seek reimbursement for fees paid to an out-of-network provider. To pursue this, you will need a “super bill,” which is an invoice with the necessary CPT and diagnosis codes. Superbills are available on a monthly basis or upon request. As with in-network benefits, a superbill will require your provider to designate a diagnosis from the DSM-V.
